We make it easy for students, teachers and health care facility workers to get their required TB test, health screen exams, & vaccinations. Payment Note: Cash, ATM, Credit Cards only NO checks and NO insurance billing. NOTE: Sorry, NO DMV exams, NO Immigration physicals, NO nursing home/MediCare exams. *All NEW weight loss patients require a New Weight Loss Patient consult appointment.
Medical Screening Exam $40 - for medical students, dental students, radiology students, nursing students, school sports physicals, some California state workers exams - call us for complete details (NO DMV exams, NO Immigration physicals, NO nursing home patients, NO MediCare evaluations.)
TB Skin Test: $30(Skin test ONLY - no chest x-ray service at Spring).
Vitamin B-12 shot: $30 for initial office visit & shot($10 each additional shot every week after that.
Vitamin B-12 Extended dose shot: (twice the regular dose) $40 for first visit and then $20 per shot after that.
LipoVite Shot:Methyl B-12 PLUS 5 other B-Vitamins & 3 additional enzymes to help clear fat and maintain the liver while on a high metabolism diet and exercise program. $45 initial visit & shot ($15 per shot each time thereafter).
Flu Shot:ONLY $15 (Now H1N1 and seasonal flu shots are combined into this 1 shot).
Vaccine PRICES arePER SHOT AS LISTED - NO EXTRA ADMIN or APPLICATION FEES Cash, Credit Cards, ATM only - sorry, NO checks and NO insurance billing. Most vaccinations are available for kids aged 11 years old and up except where noted.
Chicken Pox (Varicella) shot (4 years old and up): $115
Gardasil (HPV) shot (9 yrs old and up):$165(PER SHOT in 3 shot series)
Flu shot(4 yrs old and up):$15 each(Flu shot is now Seasonal Flu shot & H1N1 shot combined).
Hepatitis A (Hep A): $80(PER SHOT in 2 shot series)
Hepatitis B (Hep B): $80(PER SHOT in 3 shot series; 2nd shot:1 mo later, 3rd shot: 6 mo's after 1st.)
HPV (Gardasil - 9 yrs old and up): $165(PER shot in 3 shot series)
Malaria evaluation: $35 (Meet with our provider to discuss and get written prescription for malaria pills).
Meningitis (Menactra): $130(ACIP now recommends for all kids 16 years old and up)
Whooping Cough(Tdap- Adacel)shot: $60(Starting 2012 required for all kids entering 7th grade).
Yellow Fever (FY-Vax): $125 (This charge only - NO extra fees)
